Annual Service Provider Registration
Your client has requested that your service be paid by their Government Subsidised Home Care Package. All Home Care Package Providers have specific regulations and requirements they must meet as per the Aged Care Act 1997 (Cth). As your invoices are going to be paid by Government funds, the Government requires service providers (i.e. you) have certain insurances, and have signed the Charter of Rights. To meet that requirement, we need the following information from you.
Note: Invoices cannot be paid to service providers whose insurances have expired.
